Dr. J Harikishan, a Senior General Physician at Kamineni Hospitals, Hyderabad, discusses the most common signs and symptoms.

Understanding Vitamin B12 Deficiency

“Vitamin B12 is a crucial nutrient for the body, but it cannot be synthesized internally and must be obtained from dietary sources or supplements,” Dr. Harikishan explains. He further adds, “Animal products like meat, fish, poultry, eggs, and dairy are rich sources of B12. The absorption of this vitamin occurs in the small intestine with the help of intrinsic factor, a protein produced in the stomach.”

Several factors contribute to B12 deficiency, including inadequate intake, particularly among vegans and strict vegetarians who exclude animal products without incorporating fortified foods or supplements. This can lead to conditions like pernicious anemia, which impacts intrinsic factor production. The risk of B12 deficiency is also elevated in individuals with gastrointestinal disorders and those experiencing age-related declines in absorption capacity.

Research indicates that approximately 75% of the global population, comprising over 6.5 billion people, suffers from B12 deficiency. This prevalence is largely influenced by demographic variations, dietary habits, and religious practices.

Common Symptoms of Vitamin B12 Deficiency

Recognizing the symptoms of B12 deficiency can aid in early identification and intervention. Common indicators include:

  1. Fatigue and Weakness
  2. Anemia
  3. Neurological Symptoms like numbness or tingling in the extremities, difficulty walking, balance issues, and memory problems.
  4. Pale or Yellowish Skin
  5. Digestive Issues, including loss of appetite, diarrhea, or constipation.
  6. Changes in Mood, such as irritability, anxiety, and mood swings.
  7. Vision Changes

Oral Indications of Vitamin B12 Deficiency

In certain instances, vitamin B12 deficiency can manifest as glossitis—a condition characterized by inflammation of the tongue, leading to a swollen, red, and painful tongue.

Dr. Harikishan elaborates, “Common signs include a pale or smooth tongue, indicating glossitis and inflammation. Recurring mouth ulcers, known as aphthous stomatitis, can cause discomfort and disrupt oral functions. Some individuals might experience a burning or tingling sensation in the mouth or tongue. Furthermore, taste perception may be affected, resulting in diminished taste sensation or taste disturbances.”

B12 deficiency compromises the immune system, rendering individuals more susceptible to oral candidiasis, such as oral thrush caused by Candida yeast. Moreover, reduced B12 levels can result in dry mouth due to reduced saliva production, which can contribute to dental issues like tooth decay. Monitoring these oral signs is crucial for the early detection and management of potential vitamin B12 deficiencies.

High-Risk Groups

Certain demographics are at an elevated risk of developing vitamin B12 deficiency. These include:

  1. Vegans and Vegetarians: Individuals adhering to plant-based diets.
  2. Elderly Adults
  3. Individuals with Gastrointestinal Disorders
  4. Gastric Bypass Patients
  5. Chronic Alcoholics: Alcohol interferes with nutrient absorption and utilization.

“For individuals in these groups, closely monitoring B12 intake and considering supplementation if necessary is of paramount importance,” concludes Dr. Harikishan.
